Portfolios must be submitted electronically.  All components are available on this website or will be provided by the Human Resources Office.

Tool Tip We recommend that you compile your written documents in Word first, then cut and paste them into the Adobe forms when you’re ready.

Nomination Letter (web form)

      Written by unit head(s)
      One nomination letter regardless of number of unit heads, may be a joint letter
      Two pages maximum
      Letter must address:

    • demonstrated superior performance (knowledge, accountability and self management, teamwork, leadership, communication, problem solving);
    • demonstrated ability to change the organization for the better (e.g., process improvements, increased unit productivity, innovative solutions that have been implemented, etc.),
    • demonstrated and sustained commitment to professional development,  and
    • service to the University community

Supervisor’s supporting letter (web form) (if the supervisor is also the unit head, only one letter is needed)

      Written by current supervisor
      One supporting letter regardless of number of supervisors involved
      Two pages maximum
      Brief description of the candidate’s overall major areas of responsibility
      Letter must address stated award criteria

Resume (web form)

      Current resume including:
                  Education
                  Work experience
                  Honors and/or awards received for University service
                  Name of award
                  Purpose of award
                  Activity that resulted in the award

Two professional references from outside nominee’s unit, including name and address.  These should be people who can address nominee’s work quality, team work and innovations.
                             

Staff Review and Development Plans (these are on file in HR office and will be added to your portfolio at time of submission)

      Three most recent SRDPs
      All Three must have been signed by all appropriate parties at the time of review.

List of University service activities (web form)

    • Committees
    • Teams
    • Organizations
    • Other

List of professional development activities and dates completed (web form)

    1. Credit and noncredit courses completed for reasons other than completing a degree
    2. Certificate or licensure programs
    3. Workshops
    4. Training (for example, information technology)
    5. Web-based tutorials
    6. Cross-training for other positions
    7. Fellowships
    8. Internships
    9. Membership in professional organizations

Nominee’s narrative statement (web form)

      Answer the following questions.  Maximum three pages total, double spaced, 12 point font.

  1. What courses or professional development workshops have you taken over the past three years?  What did you get out of them?  How specifically, did you apply it in the workplace?
  2. Describe one or two things that you’ve done which address the strategic goals of your department(s).
  3. Name one stretch objective that you set for yourself this past year.  Did you achieve your objective?  How did you go about it and what has been the impact?
  4. Describe one thing you’ve done this year to improve a work process, reduce or eliminate unnecessary work or significantly improve the workplace.
  5. Describe something you did this year to support another unit in the Liberal Arts.
